Home Care includes skilled nursing care, physical therapy, occupational therapy, speech therapy, and home health aide services. Sparrow's home care staff provides and helps coordinate the care and/or therapy YOUR doctor orders. Along with your doctor, our staff of professionals creates a plan of care, which is a written plan for your care. It tells what services you will get to reach and keep your best physical, mental, and social well-being. Our staff keeps your doctor up-to-date on how you are doing and updates your plan of care as needed, as authorized by your doctor.

Medicare guidelines CLEARLY state: You have a choice of home care agencies. It is up to YOU to decide which agency fits your needs! Your doctor, hospital discharge planner, or other referring agency should honor your choice of home care agencies.
All care provided by Sparrow Home Care must be under the direct supervision of a physician.
